Medical Cannabis

Medical Cannabis Certification

 In order for an individual to be eligible for a qualified patient registry ID card in Kentucky (including in-state qualified patient, minor qualified pat​ient, and visiting qualified patient applicants), they must be diagnosed with one (1) of the following qualifying medical conditions:​

  • ​Any type or form of cancer;
  • Chronic or severe pain;
  • Epilepsy or other intractable seizure disorder; 
  • Multiple sclerosis, muscle spasms, or spasticity;
  • Chronic nausea or cyclical vomiting syndrome; or
  • Post-traumatic stress disorder. 


Initial certification fee of $150, then $75 each re-certification thereafter (every two months / 60 days) via telehealth.


Initial Certification MUST take place in-person. Subsequent certifications can be conducted via telehealth. Please bring relevant medical documents to initial appointment to help expedite certification for medical cannabis. 


Patient is responsible for $25 registration fee initially and annually to the state of Kentucky (separate fee paid to Kentucky) for certification card.
